Routing multiple applications through a single proxy server

Hi,
I'm currently working on a project where each of our individual machines have access to a proxy server. Each individual operates through their own port on the proxy server. There is a file on the proxy server that basically maps the individual port numbers back to the IP address of the computer sending the request. This is how the proxy server knows where to send the response. This is a little bit messy as you have to modify the file each time a new user is added.
Does anybody know a better way of doing this. Perhaps sending somethig with the request so that the proxy server knows where to send the response to. Any help would be greatly appreciated - even pointing me in the direction of a good website,

Well one solution is that you can add users in advance i.e if there are 20 users you can build a file to support 30 users
Another is you store the Ip addresses and there corresponding port Number in a database from which the file can access the information
