I am not sure whether there is open source software (a web based form � like you forms software) that can be used by a not-for-profit organization. It will primarily be used to keep track of user issue for different database used by the organization. If possible it would be preferable if it is written in Java.

Commercial software like UBB and jive usually have basic, one time up-front pricing of a few hundred dollars, and so are within the budgets of many non-profit organizations.

For many people the free bit is about libre not absense of payment.

Be careful. Although something is Open Source, it does not always mean you can freely modify the source code. There are several Open Source licenses. You need to read the agreement carefully and make sure you are able to modify source code.
Most licesnses won't restrict this, and a lot of them will allow it as long as you submit the modifications to the original developers.
