Book in the chapter 18: XSL formatting Object says...
Thus, each fo:page-sequence can contain at most five fo:flow children, one for each of the five regions on the page.

Whereas the w3c spec defines content of fo:page-sequence as

Contents:
(title?,static-content*,flow)

Moral of the story : At most one fo:flow is allowed in each fo:page-sequence.
