I have made an attempt at providing a small cheat sheet that summarizes the way exceptions are (should be) handled as per the EJB 2.0 Specification (chapter 18).

We're working on our exceptions stuff, but we don't have that cool flow chart in ours. And we don't get to use color
I think your exceptions doc will help everyone. I'm just getting ready to look at your 'responsibilities' one, but it sounds like a great summary.
