Focus for spec: API or concept?

Hi,
While reading the spec, what should the emphasis be on? The API (methods, interfaces, etc) or the concepts or both? For actual development, the API is always available and concepts (how to use what and when) are more important. But not sure if the same applies to the exam. Any clarification will he helpful.
