What do they mean???

The instructions say:

The suncertify/db directory in this distribution also contains your database file, "db.db". You may not modify this file, but you may move it if you wish.

Do they mean that the submission must include the same
file that was in the original download, despite the fact
that the program will change it during tests? The
program will change the file in some circumstances,
right?

Gregory,
Your application is required to have the functionality to modify the db.db file.
When the instructions say 'You may not modify this file', I believe they are referring to the format of the file. At least, that is how I interpreted it.

Yes, Your submission must include the same file that was in the original download. You can place the file db.db anywhere you want in your submission, instead of in suncertify/db directory.
