Perhaps this is off the ORM track, but to what extent do you cover mainframe and legacy system access in the book? Do you see this as an important issue or something that has already been addressed extensively and has limited scope?

The focus is on object - relational mappings, which means that you have to read between the lines to apply the issues to COBOL (and other procedural languages) and HDAM and other non-relational datastores.
Chapter two is about the only purely general one, as it discusses the business drivers and IT requirements that will impact your selection.
