the requirements for the user are a web browser
the requirements for the server are to have JavaMail installed http://java.sun.com/products/javamail/
there are many tutorials on how to use javamail on the web, and you just map the parameter values on to the mail fields
make sure however that the page is secure otherwise your server will be used to product gigatrons of spam mail

This is much more about JavaMail than JSP, so I'm moving it along to the Other Java APIs forum where JavaMail is discussed.
