MySQLSyntaxErrorException: Access denied

I'm getting a "MySQLSyntaxErrorException: Access denied" although I have repeatidly varified that my username/password combination are in fact correct. I'm trying to use Hibernate+MySQL+Tomcat. I've tried both MySQL5Dialect and MySQLDialect. The following is my log:

Daniel,
What is your MySQL database name? Where is it specified in the code/configuration?

Usually a URL looks something like this: (I don't remember which format mySql uses off the top of my head.)
jdbc:mysql://localhost/db
jdbc:mysql://localhost@db

Localhost is the name of the machine. Db is the name of the database on the machine.
