There are a few words on the subject HERE.

The critical bits in application design are to minimize or eliminate state in memory in any component. We push HTTP session state to the database with Websphere session persistence. Google for distributed cache solutions if you really have to keep memory state in sync. Tangosol is a good commercial solution, I'm told.

Talk to the vendors or support groups for your load balancers and server software (HTTP, Servlet container, EJB container, database, etc) about clustering options. Many of the details are product-specific.

Your hardware infrastructure folks may have some ideas about redundant power sources, blades in independent frames, redundant networks just within your cluster and so on.

Humans are always a problem. We once had a building manager cut the power to a fully loaded and running bay (football field sized room) instead of the empty bay next door.
