Are you getting an exception? If so, post it. It makes searching for problems much easier if we know where to start...

What couldn't be found?
The servlet itself or the JSP to which it forwards?

Please, don't make us pry the details from you.

Firstly: what Ben says.

Secondly, you define a display page if (responseAction.equalsIgnoreCase(SourceConstants.ADD_INC_ENTRY)) or there is an exception, but otherwise there is not response defined.
