Your first question (welcome-files) is covered in SRV.9.10 of the servlet spec. There is a link to the servlet spec in my signature.

SRV.9.12 covers the load-on-startup element.

If you're going to develop with servlets, you'll definately want the servlet spec (PDF) on your desktop so you can look things like this up quickly.

The welcome-file-list will be the file which will be called when the user access your application.

You can say it like default files, which container will look for if you havent specified any files in URL.....

If the <load-on-startup> value is greater than zero , the container will load the servlet at the time of server start up itself. Otherwise container will load the servlet at the time of first request only

It is sorta covered in the JavaRanch Style Guide.
