How to transfer data from paper to db

What is the best way to transfer information from paper to database? Can someone please shed some light on this?

The easyest one but not the cheapest : humans.

If you have a scanner you could scan it in as text (special setting on scanners) then you could manipulate the text into a format that could be easily loaded such as CSV.

Can I scan the paper directly into ASCII texts? That would be wonderful! Is that a regular feature on scanners/softwares?

Depends on your scanner. My printer (which also scans) came with software to load up that interprets the scanned items and attempts to make sense out of the input. Works fairly well for typewritten stuff. Not so well for hand written or "drawn" letters.
