Let me start of with wishing you lots of strength in overcoming this disease. Please don't take it personally that this topic isn't getting lots of responses. It's a difficult topic and not everybody is comfortable talking about it.
About your solution. I'm sure that there are many initiatives to tackle this problem. Here in the Netherlands we have the "Elektronisch patiƫntendossier". Which is basically a network of databases with all your medical information. This way hospitals can access your information. But they still need to know your name. But here everybody needs to be able to show identification so that is usually no problem. Your phone could provide such information but that can be stolen/lost/ect. Storing all your information on your phone could be a privacy risk (phone gets stolen/lost). An easy solution would be injecting a chip into your body with an identification number. But I am not really keep on injecting stuff into my body so that is also not really a solution. I think that we could discuss this problem for ever and never reach a solution that is ideal.
About the checks. I don't think we should use software to remind people to do it. I think that we should properly educate them and make them realise why it should be done. In the end it is their decision to do or not to do it. There are many thinks that we should do in order to stay healthy; eat healthy, reduce your salt and sugar intake, exercise on a regular basis, measure blood values once every x years etc. But the number of people doing that is very low. I don't do it although I know I should. My excuse is that I'm young

. But the point I'm trying to make is that we should change people instead of reminding them that they should change.
