is lang attribute node a child of title element node?

if not, they are not sibling neither,

so what are their relationship?

Attributes are different then children. It's an attribute.

One of the properties of the element in the DOM is a NodeList element that is used to store the attributes.
 
It is sorta covered in the JavaRanch Style Guide.
