Endpoint References vs Http Request URL  RSS feed

I have some doubt in Endpoint reference.

May I know how the http request URL be decided given the following endpoint reference?
Is it always set to <wsa:Address>?
If not, how the http request URL be decided generally?

(P/S: from here http://www.w3.org/TR/2005/WD-ws-addr-soap-20050215/
It says that <wsa:Address> is bind to SOAP header, but not mention anything about HTTP header)
