Regarding dynamic table generation

Hi friends,
I have one table with 5 fields and i have a one "Add" button down to that table.After Click on add buttion automatically one pop window will come and we will enter values for fields..When i submit that popup window it will store values in next row....finally i will click on submit button for page..Then values will go to Database and have to store...

Please give some Ideas with sample code As Early As Possible... Today i have to finish that work at any cost...
