How do i develop a hosted application and maintain one codebase for all the clients, but on the database side i want to have separate instances for each client. How i can setup and automate the process, for example if the model changes, i would like to replicate the change to all Database instance automatically.

ORM solutions like JPA and Hibernate can alter schemas automatically if the underlying model changes. That's some property you need to set in the config files.
