Oracle 10g - "Hanging" app modules

In our Java Enterprise Application, we are seeing that several application modules are "hanging", resulting in errors indicating that the application failed to check out an application module.
In our code, we are releasing all of the app modules.
Can anyone think of a reason why those app modules are hanging?
Any suggestions would be greatly appreciated.
